Beneficial role of skin lipid emulsion in patients with atopic dermatitis

Dry skin is associated with low stratum corneum and reduced skin barrier function. Skin medications containing skin lipids, including ceramides is potentially helpful in preserving skin barrier function and stratum corneum hydration. A study by Danby SG, et al. evaluated the benefits of a cream and equivalent lotion containing skin lipids in a multi-vesicular emulsion for the management of dry skin. It was observed that test cream and lotion both effectively increased skin hydration and showed decrease in skin dryness for at least 24 h after a single application compared to no treatment. Thus, cream and lotion containing skin lipids in a multi-vesicular emulsion helps improve skin moisturization vs. traditional emollients in patients with dry skin.

Ceramide-containing moisturizing cream effectively manages problems associated with dry skin

The intercellular lipid barrier in the stratum corneum (SC) is formed by equimolar concentrations of ceramides, cholesterol, and free fatty acids, which are arranged in the lamellar sheets. Dry skin is usually developed due to barrier disruption and intercellular lipid deficiencies, particularly ceramides. In a study by, Draelos ZD, et al. conducted a randomized, investigator-blinded, split-leg study in 53 women with dry, itchy skin. The study showed that this ceramide-containing moisturizer improved hydration of the skin and showed rapid and continual reduction in the visible signs of dry skin. Also, ceramides, cholesterol and free fatty acids were improved after the application of cream. Overall findings of this study suggested clinical effectiveness of ceramide-containing moisturizing cream in the management of dry skin.

Use of skin cleanser and moisturizer is a great combination to manage dry skin

Adequate hydration and moisturization of the skin are found to be beneficial for dry skin. Thus, daily skin care is important to keep the skin healthy. A randomized, investigator-blinded study by Kim S and colleagues was conducted to determine the role of daily skin care with a mild cleanser and moisturizer impacts skin health and patients' quality of life (QoL) in 52 patients with moderate-to-severe dry skin. Treatment arm included 39 patients who were given mild cleanser and moisturizer twice daily for two weeks and control arm included 13 patients. After two weeks of treatment, improvement in Total Clinical Score (TCS; erythema, scale and fissures), Visual Dryness Score (VDS) were reported in patients treated with mild cleanser and moisturizer. It also helped in decrease in dryness/pruritus and improved skin texture in 80% of patients from treatment arm. Quality of life also significantly improved in treatment arm vs. control arm. Thus, skin regimen with mild cleanser and moisturizer is effective in treating dry skin.

RESTORE Phase 1 Study: Effect of cream and lotion containing ceramides in patients with dry skin

In cases of dry skin, replenishing the skin lipids is the desired target for moisture retention. A double- blind intra-subject vehicle-controlled single open-application was conducted to evaluate the effectiveness of cream and lotion containing ceramides in lower legs of 22 patients with dry, atopic dermatitis (atopic eczema)-prone, skin. Total six treatment sites were considered - three per lower leg in each participant, which were treated with the test cream, the test lotion, three reference creams and no treatment as a control. Further, they were evaluated for hydration and scoring of visual dryness at timed intervals (3, 6, 12 and 24 h post-product application). Post-application of test cream and lotion, there significant increase in skin hydration and reduction skin dryness for about 24 hours after one application compared to control site. The test cream and test lotion aided for effective moisturization for 24 hours. In conclusion, the study noted that cream and lotion containing ceramides reduces the need for recurrent application of emollients (3-4 application) and also reduces the burden of managing dry skin conditions like atopic dermatitis.

Novel petrolatum-based ointment with enhanced usability effectively moisturizes and improves skin function

Petrolatum is commonly used to curb evaporation of moisture from the skin surface. However, most of the patients finds it inconvenient to use due to its stickiness, shininess, and other factors. Murakami Y and colleagues conducted a study to compared and determine the effectiveness and usability of newly developed petrolatum-based ointment (NOV® II Moisture Balm) and white petrolatum in 29 patients with atopic dermatitis or xeroderma with symptoms of dry skin. Patients were applied with the novel ointment on the right sides of the face and white petrolatum on the left side for 8-12 weeks. Scoring was considered for conditions like erythema, exudate/crusting, dryness, and itch. In addition, measurement of stratum corneum water content and transepidermal water loss (TEWL) was done, and analysis of the free amino acid and thymic stromal lymphopoietin (TSLP) levels in the stratum corneum were done before and end of the study. There was significant increase in the water content and the free amino acid levels in the stratum corneum. With respect to usability of the ointment, majority of patients described that newly developed petrolatum-based ointment had better spreadability and was less sticky compared to white petrolatum, and these patients wanted to continue its use. Overall findings suggested that NOV® II Moisture Balm demonstrated comparable effectiveness for moisturization; but, had an advantage for better usability compared to white petrolatum in the management of dry skin.
